Answer:-

P             =            12 bits

Off set =             5

Let the number of bits required                                =             x
so,          2^x         = 4096
so,          x              =12

Offset   = 5

2^p        = 4096

2^p        = 2^12

p             =12
2^d        = 32
2^d        =2^5

d             =5
