Scenario

Pacific Batteries Company has received a complaint from its regional office that the new lot of batteries is not working properly. After checking it was identified that there is no problem with the batteries, the technicians at regional office are attaching the wrong wire connections. Upon enquiring it is identified that the previous lot of batteries has “+ve” and “-ve” signs on the battery terminals and usually “+ve” terminals are of red color and “-ve” terminals are of green color. But in the new lot there were no signs and it was clearly written in the user manual that green terminal is of positive charge and red terminal is of negative charge. When the technicians were asked why they are not reading the user manual, the typical response was, “We never read those user manuals anyway”.

After reading this case identify whose mistake is this? How can this problem be solved in future?
